Improve your organization's medication history and reconciliation processes and thus reduce the incidence of medication errors.
This topic will look at the role medication histories play in ensuring that patients stay safe in receiving medical care. We will look at the history of medical recordkeeping after the widespread adoption of at-home medications, the elements of a good medication history (for providers), and methods of storage and utilization of medical record history information (interaction databases, research databases, and conventional physical tools used for patient safety). This material will also look at what happens when medication histories go wrong, looking at case examples to provide you with a look at best practices going forward.
